Richmond County Hospice takes referrals from any source: physicians, family members, nurses, discharge planners,
friends or self-referrals.  Our admissions team works with the patient, the patient's caregivers and the patient's physician to decide if the patient is ready for hospice services and get the authorization to begin
services.
